Tretinoin is a topical medication commonly used for the treatment of acne and certain skin conditions. It is a derivative of vitamin A and belongs to a class of medications called retinoids. Tretinoin works by increasing the turnover of skin cells, which helps to prevent the formation of acne and promotes the growth of new, healthy skin.

Here are some key points about tretinoin:

    1. Acne treatment: Tretinoin is primarily used to treat acne. It can help unclog pores, reduce inflammation, and promote the shedding of dead skin cells, which can help prevent the formation of new acne lesions.
    2. Besides acne, tretinoin can also be used to treat other skin conditions such as fine wrinkles, rough skin texture, and certain types of dark spots or hyperpigmentation.
    3. Tretinoin is available by prescription only and comes in various formulations, such as creams, gels, and lotions. The strength and frequency of application may vary depending on the condition being treated and the individual’s skin sensitivity.
    4. When starting tretinoin, some individuals may experience skin dryness, redness, itching, or mild peeling. These side effects are usually temporary and can be managed by gradually introducing the medication and using moisturizers and sun protection.
    5. Tretinoin can make your skin more sensitive to sunlight. It is important to use sunscreen with a high SPF and protect your skin from direct sun exposure while using tretinoin. Sunscreen should be applied daily, even on cloudy days.

Topical clindamycin is an antibiotic that is commonly used for the treatment of acne and certain types of bacterial skin infections. Here is some important information about topical clindamycin:

    1. works by inhibiting the growth of bacteria. It binds to bacterial ribosomes, which prevents the synthesis of bacterial proteins.
    2. Topical clindamycin is often prescribed for the treatment of acne. It helps reduce the number of acne-causing bacteria on the skin and decreases inflammation associated with acne.
    3. Clindamycin is often prescribed in combination with other acne medications, such as Tretinoin, Azelaic acid, and benzoyl peroxide to enhance effectiveness and reduce the risk of bacterial resistance.
    4. Some common side effects of topical clindamycin include skin dryness, itching, redness, and peeling. These side effects are generally mild and transient. Allergic reactions are rare but possible, and if severe symptoms occur, medical attention should be sought immediately.

Azelaic acid is a topical medication that is commonly used for the treatment of acne and certain skin conditions. It is a naturally occurring acid that is derived from grains like wheat, barley, and rye. Azelaic acid has antibacterial and anti-inflammatory properties, which make it effective in reducing acne lesions and improving the overall appearance of the skin.

Here are some key points about azelaic acid:

Acne treatment: Azelaic acid is often used to treat mild to moderate acne. It works by reducing the growth of bacteria on the skin and preventing the accumulation of dead skin cells, which can clog pores and lead to acne breakouts. Additionally, it helps to reduce inflammation associated with acne.

Hyperpigmentation: Azelaic acid is also used to treat hyperpigmentation, which refers to dark spots or patches on the skin caused by excessive melanin production. It can help fade hyperpigmentation by inhibiting the enzyme responsible for melanin synthesis.

Rosacea treatment: Azelaic acid is approved for the treatment of rosacea, a chronic skin condition characterized by facial redness, flushing, and acne-like bumps. It helps to reduce redness and inflammation associated with rosacea and can improve the overall appearance of the skin.

Side effects: Azelaic acid is generally well-tolerated, but some individuals may experience mild side effects such as skin irritation, dryness, or itching. These side effects are usually temporary and resolve with continued use. If severe or persistent side effects occur, it is advisable to discontinue use and consult a healthcare professional.

Adapalene is a topical retinoid medication that is commonly used for the treatment of acne. It belongs to the same class of medications as tretinoin and works by promoting the turnover of skin cells and preventing the formation of acne lesions.

Here are some key points about adapalene for acne treatment:

    1. Adapalene helps to regulate the growth and differentiation of skin cells. It works by normalizing the shedding of dead skin cells and preventing the formation of comedones (clogged pores). It also has anti-inflammatory effects, which can help reduce the redness and swelling associated with acne.
    2. Adapalene is effective in treating different types of acne lesions, including whiteheads, blackheads, and inflammatory pimples. It can be used as part of a comprehensive acne treatment regimen, which may include other medications like benzoyl peroxide or oral antibiotics.
    3. Adapalene not only helps to clear existing acne but also helps to prevent new acne breakouts from forming. It can improve the overall appearance of the skin by reducing the number and severity of acne lesions, as well as minimizing post-inflammatory hyperpigmentation (dark spots) that can occur after acne heals.
    4. Adapalene is usually applied once daily in a thin layer to the affected areas of the skin. It is important to follow the instructions provided by your healthcare professional or the product label. It may take several weeks of consistent use before you start seeing noticeable improvements in your acne.
    5. Adapalene may cause some initial skin irritation, such as redness, dryness, or peeling. These side effects are usually mild and temporary, and the skin typically adjusts to the medication with continued use. However, if you experience severe or persistent irritation, it is advisable to reduce the frequency of application or discontinue use and consult a healthcare professional.
    6. Sun sensitivity: Adapalene can make your skin more sensitive to sunlight, so it is important to use sunscreen with a high SPF and protect your skin from direct sun exposure while using this medication.

Niacinamide, also known as vitamin B3 or nicotinamide, is a water-soluble vitamin that offers various benefits for the skin. It is commonly used in skin care products and topical formulations due to its numerous positive effects. Here are some key points about niacinamide:

    1. Skin barrier function: Niacinamide helps to improve the skin’s barrier function, which is essential for maintaining healthy, hydrated skin. It enhances the production of ceramides, a type of lipid that helps to retain moisture and protect the skin from external irritants.
    2. Moisturization: Niacinamide has humectant properties, meaning it helps to attract and retain moisture in the skin. This can help improve hydration levels, reduce water loss, and alleviate dryness and flakiness.
    3. Minimizes pores: Niacinamide can help to minimize the appearance of pores by regulating sebum production and reducing excess oil production, it can make the pores appear less noticeable.
    4. Reduces inflammation: Niacinamide has anti-inflammatory properties that can help calm and soothe inflamed or irritated skin. It can be beneficial for those with acne or sensitive skin conditions like rosacea.
    5. Hyperpigmentation: Niacinamide has been found to inhibit the transfer of melanin (pigment) within the skin, making it useful in addressing hyperpigmentation issues such as dark spots, age spots, and melasma. It can help even out skin tone and improve the overall complexion.
    6. Antioxidant effects: Niacinamide possesses antioxidant properties, which means it can help protect the skin from oxidative stress caused by free radicals. This can help prevent premature aging and damage caused by environmental factors like pollution and UV radiation.
    7. Compatibility: Niacinamide is generally well-tolerated by most skin types and is considered non-irritating. It is suitable for various skin conditions, including acne-prone, sensitive, and aging skin.

The fundamentals of
your Oral treatment

Oral medications are prescribed for the treatment of acne when topical treatments alone are not sufficient. These medications are typically used for moderate to severe acne or for cases where acne is not responsive to other treatments.
Combining oral and topical medication can be very effective in treating acne and avoiding the development of scars.
Here are some common oral medications that we prescribe at ZappedSkin for acne.

Lymecycline is a broad-spectrum antibiotic that is commonly used for the treatment of acne. It belongs to the tetracycline class of antibiotics and is primarily prescribed to reduce the population of bacteria that contribute to acne breakouts.

Here are some key points to know about lymecycline:

    1. Mechanism of action: Lymecycline works by inhibiting the growth of bacteria, including Propionibacterium acnes, which is a bacterium associated with acne. By reducing the number of bacteria, lymecycline helps to control inflammation and prevent the formation of new acne lesions.
    2. Prescription and dosage: Lymecycline is available in tablet form and is typically taken orally. The duration of treatment may vary depending on the severity of the acne. Typically, a dermatologist will recommend a 3 to 6 months treatment course.
    3. Effectiveness: Lymecycline has been found to be effective in treating acne, particularly inflammatory acne lesions. It can help reduce the number of acne lesions, control breakouts, and improve the overall appearance of the skin. However, the response to lymecycline can vary among individuals, and it may take several weeks or even months to see significant improvement.
    4. Side effects: Like other antibiotics, lymecycline can have potential side effects. Common side effects may include gastrointestinal symptoms such as nausea, vomiting, diarrhea, or abdominal discomfort. It can also cause photosensitivity, making the skin more sensitive to sunlight. Less commonly, allergic reactions or the overgrowth of certain bacteria (such as Clostridium difficile) can occur. If you experience any concerning side effects, it is important to consult your healthcare professional.
    5. Precautions: Lymecycline should not be taken by individuals who are allergic to tetracycline antibiotics or have a history of severe liver disease. Lymecycline must not be taken if you are pregnant or breastfeeding.

Erythromycin is an antibiotic that is sometimes prescribed for the treatment of acne. It belongs to the macrolide class of antibiotics and is used to reduce the population of bacteria involved in acne breakouts, such as Propionibacterium acnes.

Here are some key points to know about erythromycin for acne:

    1. Mechanism of action: Erythromycin works by inhibiting the growth of bacteria. It does this by interfering with the protein synthesis process in the bacteria, thereby reducing their ability to multiply and causing them to die off. By targeting the acne-causing bacteria, erythromycin helps to control inflammation and improve acne symptoms.
    2. Usage: Erythromycin for acne is typically available as an oral medication or as a topical gel, lotion, or solution. The specific form and dosage prescribed will depend on the severity of the acne and the recommendation of your dermatologist.
    3. Effectiveness: Erythromycin can be effective in treating acne, especially for mild to moderate cases. It is often used as an alternative or in combination with other acne treatments, such as topical retinoids or benzoyl peroxide.
    4. Side effects: Like any medication, erythromycin can have potential side effects. Common side effects may include gastrointestinal symptoms such as nausea, vomiting, diarrhea, or abdominal discomfort. Less commonly, allergic reactions or skin irritation may occur with topical formulations.
    5. Precautions: Erythromycin may interact with certain medications, so it is important to inform your healthcare provider about any other medications or supplements you are taking. Additionally, erythromycin should not be taken by individuals with known allergies to macrolide antibiotics.

Doxycycline is an antibiotic commonly used in the treatment of acne. It belongs to a class of antibiotics called tetracyclines and works by inhibiting the growth of bacteria that contribute to the development of acne.

When used for acne treatment, doxycycline helps to reduce inflammation and control the population of bacteria on the skin. It is often prescribed for moderate to severe cases of acne that do not respond well to topical treatments alone.

    1. Mechanism of action: Doxycycline targets and inhibits the growth of bacteria, particularly Propionibacterium acnes (P. acnes), which is a bacteria commonly associated with acne. Doxycycline has anti-inflammatory properties that contribute to its effectiveness in treating acne. It inhibits the production of certain pro-inflammatory molecules and enzymes, such as interleukin-1 (IL-1) and matrix metalloproteinases (MMPs), which are involved in the inflammatory process associated with acne.
    2. Usage: Doxycycline for acne is typically available as an oral medication and it is usually prescribed to be taken once or twice daily for a 3 to 6 months period.
    3. Effectiveness: Doxycycline can be effective in treating acne, especially for mild to moderate cases. It is often used as an alternative or in combination with other topical acne treatments (creams).
    4. Side effects: Like other antibiotics, Doxycycline can have potential side effects. Common side effects may include gastrointestinal symptoms such as nausea, vomiting, diarrhea, or abdominal discomfort. It can also cause photosensitivity, making the skin more sensitive to sunlight. Doxycycline can cause permanent discoloration of developing teeth in children under the age of 8. It can also affect bone growth in children if used for prolonged periods.
    5. Precautions: Doxycycline should not be taken by individuals who are allergic to tetracycline antibiotics or have a history of severe liver disease. Lymecycline must not be taken if you are pregnant or breastfeeding.

Spironolactone can be used off-label as a treatment for acne, particularly in cases where hormonal factors contribute to the development of acne. Here’s some information regarding the use of spironolactone for acne:

    1. Hormonal acne: Hormonal imbalances, particularly an increase in androgen levels (such as testosterone), can lead to the development of acne. Spironolactone, with its anti-androgenic properties, can help counteract the effects of these hormones and improve acne symptoms.
    2. Mechanism of action: Spironolactone works by blocking the androgen receptors in the body, reducing the effects of androgens on the skin. By doing so, it can decrease oil production, prevent the formation of clogged pores, and reduce the occurrence of acne breakouts.
    3. Effectiveness: Spironolactone has been found to be effective in managing acne in some individuals, particularly in cases of hormonal acne or acne that does not respond well to other treatments. However, its use for acne should be discussed with a dermatologist.
    4. Treatment duration: The duration of spironolactone treatment for acne can vary. It usually takes several months to see noticeable improvements in acne symptoms. The treatment may be continued for an extended period to maintain the results.
    5. Combination therapy: Spironolactone is sometimes used in combination with other acne treatments, such as topical medications or oral contraceptives. This combination approach can be more effective in managing acne, especially when hormonal factors are involved.
    6. Side effects: While spironolactone is generally well-tolerated, it can have side effects. Common side effects may include increased urination, dizziness, fatigue, breast tenderness or enlargement, and irregular menstrual periods. It can also affect potassium levels in the body, so regular monitoring may be required. Patients on Spironolactone need regular blood tests to check their kidney function and potassium levels.

Isotretinoin, also known by its brand name Roaccutane, is a medication primarily used for the treatment of severe forms of nodulocystic and acne conglobate. Isotretinoin is typically prescribed when other acne treatments have failed or in cases where acne is causing significant physical or psychological distress.

In the UK, Isotretinoin can only be prescribed by a dermatologist or by a team that is directly supervised by a dermatologist

Isotretinoin belongs to a class of drugs called retinoids, which are derived from vitamin A and it works by:

  • Reducing the production of oil in the skin,
  • Decreasing the size of sebaceous glands, and
  • Preventing the clogging of pores.

This helps to prevent the formation of new acne lesions and promotes the healing of existing ones.

Isotretinoin is usually prescribed over a period of 6 to 8 months depending on the patient’s weight and tolerance.

Due to its potential side effects, isotretinoin is generally prescribed under strict monitoring including:

  • Pregnancy prevention program for female patients of childbearing age.
  • Regular follow-ups with a consultant dermatologist
  • Blood tests, at least 3 blood tests during the course of the treatment which typically lasts from 6 to 8 months

Common side effects include dry skin, chapped lips, dry eyes, nosebleeds, and muscle aches. It can also cause more severe side effects such as birth defects if taken during pregnancy, depression, liver damage, and an increase in blood lipid levels. Therefore, it is crucial to use effective contraception while taking isotretinoin and to undergo regular monitoring and follow-up with a dermatologist.
